to help ensure that their long term financial needs are secured.
PLAN of Massachusetts works with families who wish to plan
for the future needs of their loved one with a life-long disability.
The work we do with both individuals and families includes
providing education and information about alternatives for
preparing for the future.
We provide families who enroll in the PLAN of
Massachusetts Third Party Pooled Trust assistance in developing
an individualized and comprehensive Life Care Plan.
The Life Care Plan provides a road map for
continuing care of the family member with a life long disability.
It identifies the legal, financial, emotional, and social
needs that need to be met when family caregivers are no longer
able to provide care and support to their loved one with a
life-long disability. Our Planning services help ensure families
that their loved one with a disability will continue to receive
quality care and services after family caretakers are no longer
available.
The individualized Life Care Plan
responds to real-life situations today and anticipates those
that may occur tomorrow. It is flexible, highly individualized,
and it recognizes the changing needs of people and alters
the plan to meet those needs.
The process of developing the Life Care
Plan includes identifying and setting aside financial
resources for its implementation.
